Evaluation of the RN Transitions into Practice Residency Model for Long Term Care Program

The Heldrich Center is evaluating a 12-month nurse residency model in NJ Long Term Care (LTC) facilities for new RN graduates. The program identifies that in contrast to acute care environments, nurse residency programs have not been offered in long term care environments, contributing to high turnover and attrition rates, and reduced care for LTC residents. This program will address current problems in LTC facilities and educate 50 preceptors and 50 new RN graduates with the goal of improving care and reducing re-hospitalization rates for LTC residents. This project is funded by Federal Civil Monetary Penalties.
